Nurses and other healthcare workers applying for a UK Health and Care Worker visa typically need a B1 SELT — a Secure English Language Test at B1 (Intermediate) level. The test assesses speaking and listening only; there is no reading or writing component for visa purposes.
SELT vs OET — important distinction
The SELT is for your UK visa application. The OET (Occupational English Test) is for professional registration with bodies such as the NMC (nursing) or GMC (medicine). These are different requirements. Many healthcare professionals need both. Check with your employer and the relevant regulatory body.
The B1 SELT for a Health and Care Worker visa is the same test taken by other visa applicants — it is not a medical English test. You are assessed on general English at B1 level: fluency, vocabulary range, grammatical accuracy and pronunciation. The examiner does not ask clinical questions.
However, nurses and healthcare workers often have strong passive English from reading medical documentation but weaker spoken fluency. The SELT specifically tests spoken production — which means active speaking practice is more valuable than reading preparation.

SELT test centres have limited availability. Healthcare worker visa timelines are often tight. Book the test date first, then plan your preparation around it.
Focus on spoken fluency, not medical English
The test is about general English at B1 level. You do not need medical vocabulary. You need to speak fluently about everyday topics — work, family, hobbies, your local area.
Practise developing answers, not just giving them
Many healthcare workers give correct but very short answers. The SELT rewards candidates who develop their points with reasons and examples. Practise extending every answer to at least three sentences.
Work with a SELT specialist, not a medical English tutor
A general medical English teacher will prepare you for OET or clinical communication. For the SELT you need someone who knows the specific test format and marking criteria.
